Last 4 of SSN:

Enter the last four digits of your Social Security Number (SSN). This should only be filled out if you are conducting business with the State

as an individual, a sole proprietor or a Single Member Limited Liability Company (LLC).

Core-CT Vendor ID Number:

If you have conducted business with the State previously and know your ten digit Core-CT Vendor ID Number enter it here.
